SUMMARY:

The Case Manager will plan and coordinate the care of clients receiving home care and intermittent care including making skilled nursing visits and supervising Home Health Aides. Other duties include:

* Assure that the emotional and spiritual needs and the safety and physical comfort of the client are satisfied.
* Perform nursing procedures.
* Participate in preparing and implementing the Plan of Care and client care planning.
* Teach clients, family members, and auxiliary nursing personnel proper health maintenance care.

Education:

* Graduate of a school of professional nursing. Currently licensed to practice in state where employed.

Experience:

* Minimum of 1 year experience as an RN; PHN and home care experience preferred.
* Verification of experience and skills required to practice in specialty areas (e.g., pediatrics, oncology, rehabilitation, IV therapy, maternal/child, etc.)
